Estimado colega, lo que ocurre en la sarna es, que la muerte de los ácaros adultos produce infección e inflamación en la piel. Al estar los parásitos, en "galerías", también infectan he inflaman los folículos pilosos (alopecia). Es normal que en casos muy parasitados, al comenzar el tratamiento acaricida, el paciente se observe mucho mas  infectado e irritado, pero esto será en las primeras 2 o 3 semanas. Personalmente acompaño el tratamiento de terapia inmunoestimulante y en casos de ser necesario también antibiótica.
En segundo lugar quisiera agregar que histologicamente cualquier avermectina, se describen con gran inflamación y reacción adversas inmunitarios en equinos. Es por eso que ninguna viene indicada inyectable. Personalmente solo la aplico inyectable en casos críticos (sarna o trastornos alimentarios) y nunca he tenido problemas. Debo agregar que de todas formas en el control parasitario de rutina las utilizo en forma oral.
